Melanotan-2 is a synthetic cyclic analogue of α-MSH, created for pigmentation enhancement. However, early trials revealed strong proerectile activity, shifting focus to sexual medicine. MT-II binds melanocortin receptors, especially MC4R in the CNS, leading to erections and increased libido. Studies showed no effect on penile smooth muscle directly, but erections depended on nitric oxide pathways. Human trials demonstrated erections in over 60% of men with erectile dysfunction. MT-II's central action and resistance to enzymatic breakdown position it as a potent agent for treating sexual arousal disorders.
